La diabetes mellitus tipo 1 es una enfermedad crónica de origen autoinmune. En la actualidad aumenta de manera constante la incidencia de esta afección. Hasta ahora los mayores problemas clínicos siguen siendo las complicaciones crónicas asociadas a esta enfermedad, que influyen en la calidad y esperanza de vida en este grupo de enfermos.2 Por este motivo, se siguen buscando e investigando los factores de riesgo para la angiopatía diabética. Entre los factores de riesgo tradicionales se encuentran sobre todo la hiperglucemia y la diabetes no corregida metabólicamente, además de la hipertensión arterial, la displidemia y el tabaquismo.3,4 Vale la pena subrayar que la mayoría de los factores de riesgo mencionados arriba se puede modificar mediante la administración de fármacos, la introducción de una dieta adecuada y de la actividad física, lo que conlleva varios beneficios. Sin embargo, el abandono del hábito tabáquico resulta ser más complicado y difícil de conseguir para los enfermos.5,6 El tabaquismo constituye un problema importante también en la población de enfermos con diabetes mellitus tipo 1. La cantidad de fumadores en este grupo es similar a la de la población general.7 De los estudios propios se desprende que cada 1 de 3 adultos con diabetes mellitus tipo 1 es fumador.8 El tabaquismo en este grupo tiene una influencia considerable en el curso de la enfermedad.

Tabaquismo y período de remisión parcial

Al comienzo de la diabetes mellitus tipo 1 puede producirse una remisión parcial de la enfermedad. Esta remisión está relacionada con la recuperación de la función de las células ß que sobrevivieron el proceso autoinmune inflamatorio y con la regeneración parcial de las células que producen insulina. La remisión se caracteriza por el mantenimiento de la normoglucemia, al usar dosis pequeñas o nulas de insulina exógena, y un resultado positivo de la prueba del péptido C. Se han descrito varios factores que de una manera positiva o negativa influyen en el período de remisión clínica parcial. Entre los factores que acortan la duración de la remisión o incluso la inhiben se cuenta el tabaquismo.34 La aparición de la remisión parcial en la diabetes mellitus tipo 1, a su vez, tiene un impacto importante en la reducción del riesgo de aparición de complicaciones crónicas de esta enfermedad.35
