Is it possible to avoid insulin therapy in patients with long-standing type 2 diabetes?

René Rodríguez-Gutiérrez, MD: People tend to think that as diabetes progresses, no beta-cell function remains and in the end they will need insulin, but the truth is that there are many patients who have had type 2 diabetes for over 20 years that are still on one or two medications.

I think the key point here is to have good lifestyle interventions early and later in the course of diabetes. Eating well is very important and so is, of course, exercise, but I think there is no reason to think that all patients with type 2 diabetes will end up using insulin.
