Which insulin regimen should be preferred in the elderly with type 2 diabetes?

René Rodríguez-Gutiérrez, MD: I think here the answer is to keep it simple with the lowest possible risk of hypoglycemia. I would say use a basal long-acting insulin, whichever the patient can afford. Begin with maybe 10 to 20 units and try to titrate the dose, but be very conscious that what we want to avoid is hypoglycemia.
